Mandatory Checklist

Income:
  • W-2 (All employers)

  • W-2G (Gambling winnings)

  • 1099 Forms (1099-R, 1099-NEC, 1099-MISC, 1099-INT, 1099-DIV, 1099-B)

  • SSA-1099 (Social Security Benefits)

  • Any other income not reported (tips, overtime, etc.

Home & Property:
  • Mortgage interest (Form 1098)

  • Property taxes (Form 1098 if impounded account)

  • Vehicle Registration Fees

  • Energy Efficiency (requires QMID from Manufacturer)

  • Solar installation documents

  • Rental property income and expense records

  • Sale of home (Form 1099-S)

Expenses:
  • Medical and dental (out-of-pocket only)

  • Work-related

  • Charitable contributions (cash/non-cash)

  • Child Care (provider name, address, phone number and EIN or Social Security Number, amount paid per child, per location)

Other Important Items:
  • Estimated or quarterly tax payments made during year.

  • IRS Identity Protection PIN (if issued)

  • New vehicle purchase contract (sales tax, registration and loan interest)

  • Bank account and routing number for direct deposit or payment of taxes due.

  • Prior year tax return (new clients only)

Education Expenses:
  • Student loan interest, Form 1098-E

  • Tuition payments, Form 1098-T

  • 529 plan distributions, Form 1099-Q
